Whether it works or not could depend on the modell of USB board and (extension) cable. Try the rift directly attached to other USB ports of your computer - if you can't get it working at all it could be a failure of the HMD.
In that case get in contract with Oculus support - otherwise you will need to get you a different cable / USB card.
The weird thing is even though it is not being recognized by the system when I go to the diagnostic tool from Oculus it generate me the info below on USB logs, so it can recognize that I have a oculus device ( the only one conected is the headset) but when in the oculus app it does not find it : --------------------------------------------

Try RIFT HDMI>DP Adapter 4K instead of direct to HDMI. Also that OculusVR is not set to Run As Program By Admin in it's Properties/Compatibility tick box.
I have recently purchased Oculus Rift. It starts well but goes blank within 5 minutes, freezing the laptop. Please solve the problem as early as possible. Make me happy. At the moment I am feeling like a MAD that I purchased OCULUS RIFT. Wanted to share the VR experience with entire family. And Oculus just ruined the happy atmosphere.
Windows 10 system doesn't appear to have RiftDisplay.inf installed. I Dont Understand What to do ive tried everything. Something is wrong here and multiple people are reporting this issue please help us fix this i waited several years to buy a rift to kink out all the bugs and i cant even use it once come on now this is not right.
Bought a Rift + Touch, setup last Thursday (June 21/2018) everything worked fine until this morning. Started the app which forced an update that wont install. Uninstalled via add/remove programs, re-downloaded the setup.exe from oculus/setup. ran the setup to install and went to configure the rift/complete setup and am stuck on the "Put on your Headset and continue setup in VR. screen is black, white LED light above the lenz. colour me very unimpressed..... as I have already purchased a few hundred $ worth of products from the store. and the community census seems to be a complete re-install of everything on my PC?.... If this last update was released in April, why was it not the version released when I ran the setup 4 days ago... I would appreciate a real answer to this issue. nothing in my PC setup has changed in the 4 days I have been using the Oculus rift until some forced update from Oculus last night which renders the app and headset unusable.
again... a REAL answer to this problem would be appreciated.
I to have recently have had the black out issues but can hear the game or home in the back ground, i don't think it has anything to do with the rift itself but more to do with some issue with software in terms of updates either Windows or Nvidia etc. I already have my ticket through and logs have been sent so waiting for an answer. Really frustrating as you spend a fortune on the VR set and the upgrades to the PC for it all to work which it did until now. I wonder if the VIVE has similar issues?
Hi. Today and yesterday i've been having constant issues with the hdmi signal. After starting a game, the image disappears from the HMD after anything from 2 minutes to an hour of gaming. In the case of Elite Dangerous, the game keeps running on my monitor, but the image in the HMD is replaced by an Oculus logo. When playing Hellblade yesterday, i got a fatal error and a hard game crash every time instead. Latest NVIDIA drivers installed. GPU: 1080TI Any help would be greatly appreciated. Log file attached. I am having the same issues. Everything ran fine and as of the last couple weeks games are crashing to the desktop anywhere from 10 minutes to an hour into a gaming session. I have monitored my hardware via MSI afterburner and everything is running well and cool. I have had the complete setup running fine for nearly a year. Now out of the blue just constant issues. Had the blackscreen for some time now, only bought it a month ago and worked fine. started it one day after my brother had removed the monitor and that was that. Please help
Looks like there could be USB issues there. Can you try updating USB drivers for your motherboard? Otherwise, you can also try uninstalling the USB driver in Device Manager and using Microsoft default.
Had the blackscreen just now, after updating oculus firmware after not using it for about one and a half months. Can't even set my sensors up, crashes after 5-10 secs in the VR part of the sensor setup

So an update after trying different scenarios which people in the forums spoke of and after speaking to Oculus support, sending the logs over for them to look over. Even they do not know whats wrong so they want me to send the rift back and in turn i will be sent a refurbished one.

After all did a clean install and still only had a black screen in the rift, so by than was thoroughly annoyed as you might imagine.
Solution for me was however surprisingly simple, might help somebody else so will post anyway.
While using the AMD 390 card I had the HDMI port shared with either a monitor or the rift connected. To be able to do so and not have to dive behind my desk and computer each time I had an HDMI extension cable inserted into the video card port and with the end of the extension cable on the top of my desk.
So whenever switching between monitor and rift I had easy access to change the cables from the top of my desk. Worked like a charm while still using the AMD videocard never any issues.
As the 1080 has more output ports I no longer need to switch monitor and rift using the same port. But never realized but still had the rift attached via the HDMI cable extension connected to the nvidia HDMI port.
Removed the HDMI extension cable , directly connected the rift to the 1080 HDMI port and immediately vision popped up.
Disconnected the rift and checked if there somehow was an issue with the HDMI extension by attaching a monitor and that still worked fine. Replaced monitor for the rift vision went black again. So the used HDMI extension cable in combination with nvidia was what kept messing things up while it never was a problem with the AMD card.
Just to be complete the clean windows 10 install was including march 2018 updates and I used most recent nvidia drivers (398.something) .
Rift works like a charm again an by now very happy with the additional VR performance boost from the 1080.
Hi. I just wanted to share my findings, in case someone else has my particular problem/setup (vega gpus in crossfire) .
Only for crossfire/dualgpu users
Issue: Black screen showing mouse on monitor, all black in rift. If I pulled the rift, the game would show on the monitor.
Note: After updating windows to 1809 it worked once. But error was back the day after.
Solution: My solution was to plug both the monitor and the rift into the lower gpu, the one furthest from the CPU. This is of course counter intuitive, as that is not the main GPU. So, in doing this I was no longer able to see POST, however, when I eventually got into windows, EVERYTHING WORKED !!
Here me logs... Display would flash the Oculus home for 1 second then go black...but now wont turn on at all....jsut a few static flashes here and there.
I have latest Nvidia drivers, and i bought the rift s yesterday, and vr headset's display just shows me a black screen, i don't even know how is the headset when it works, i just have the sound working. It's really sad. I'm always in the "Set up in VR" step in Oculus app. Help please?
Edit: I did a rollback for Nvidia Drivers to an older version (not 388.59 because i can't, and i don't know why) Now Oculus Works, but only for less than a minute, and fades to black again. It does the same thing when i plug it out and in again.

Today and yesterday i've been having constant issues with the hdmi signal.
