OS: Win7 64-bit
GPU: Zotac GTX 980
Video Drivers: 344.65
CPU: i7-950 @ 3.07ghz
Monitor(s): 3x 144hz Monitors, 2 using display ports (primary, secondary), 1 using dvi (tertiary)
Connection(s): DK2 (4th display) plugged in via HDMI port
Other: Alot of peripherals - attaching dxdiag report (7 usb devices that includes logitech gaming keyboard, mouse, camera, not including DK2 + camera)

I should add that I have the positional tracker and HMD connected to a powered USB hub as I could not get the camera to turn on using my onboard USB ports. I believe it is due to the hubs having to supply power to all of my other devices.

edit: updated monitor specs with current hardware
