Screen flash ingame (Grey/Blue) Pics and Video *EDIT* (FIXED with solution)

So I have been encountering this issue and it's gotten to the point that I am going crazy trying to fix this. I have searched all over and I have changed many thing to correct this. Today I took some screen shots and recorded it. Many times it happens when I pull the trigger to shoot a gun and the screen will either partially flash and I'll see my touch controllers, (picture on the left) or it will completely wash out. (picture on the right)
I was told it can be CPU contention and I followed the steps to have the 3D physics in the nvidia control panel set to GPU, I run the game on priority, and I have some settings off as recommended. Mostly in this game (onward) however I have seen it in other games, I mainly play onward. I am not sure what else to do.

Rift setup: 3 sensors, 2 sensors have a 6ft USB 3.0 extension, headset has a 6ft 3.0 USB and HDMI extension.
I record using OBS and a Logitech 920 webcam and record in window mode with Oculus Mirror

Please someone help

*EDIT* SOLUTION BELOW!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

*UPDATE November 1st 2017*
So after speaking to someone and having them check out my oculus system log they saw massive tracking issues. We came to the conclusion that my CPU was overloaded. (I was running 14 things on the 8 usb's on the CPU with a 3.0 USB hub.
So I bought this.....

TADA!!!!!!!!!!! Problem solved! Now I understand after also reading this article. http://www.pcgamer.com/oculus-outlines-how-room-scale-vr-can-overload-your-usb-controller/
When I was using 2 sensors everything was fine, the day I added the third, I lost my mind! Now the war is over! Thanks guys!

Tried without the extension cables? If u have a spare pc I'd try connect it to that to rule out the VR hardware.

I'm gonna try without the extension today, few buddies told me that's what happens to them when they lose tracking. I shouldn't be losing tracking unless the signal isn't getting their fast enough because of an extension. I have another older build but no power supply or Windows to run it in to test it 😞 I'll update my findings.

Tried it without the headset extensions, no difference, tried it with and without super sampling, got OTT and tried it with all suggested settings from other people on other games and forums. Happens in other games too. When it happens my sensors all flash on the screen too and so do my controllers as if it loses tracking. Extremely frustrating.

Looks like the flash you see might by how the game handles when you go outside the tracking bounds. It's possible there is some other issue on your system, for example with USB connection, that causes the sensors to go offline periodically and lose the tracking. I would try plugging some of the sensors and the headset into USB 2.0 instead. You can also get a USB 2.0 extension (don't go longer than around 3 meters or 10 feet) or use your existing cables but just plug them in on USB 2.0. You may also want to go through the sensor setup if you haven't done that in a bit. Hope that helps.
